Performs a variety of duties, projects, and program management to support the Community Wellness & Bariatric Services functions. Assists with program planning and delivery for community events, initiatives, and wellness programs. Markets services and other programs to providers in the area and surrounding counties. Provides nutritional counseling and education to the community through onsite programs, and at health fairs and local events. Promotes community activities that work toward the objectives set forth in the Community Health Needs Assessment.
As part of the Bariatric Services team, evaluates patient's appropriateness for surgical weight loss. Provides nutrition education to help bariatric surgery candidates understand the lifestyle necessary pre and post-surgery to achieve healthy outcomes. Leads support group education meetings on monthly basis.
Minimum Qualifications Work Experience Two years of clinical experience in an acute hospital setting preferred. Previous experience providing outpatient medical nutrition therapy counseling preferred.Education Bachelor’s degree in dietetics and a completed Internship program required.
Certifications/Licensure Licensed in the state of Kentucky and Commission on Dietetic Registration (Academy of Nutrition and Dietetics) required. Markets Medical Nutrition Therapy services and other programs to providers in the area and surrounding counties including in person and virtual events as well as social media and other marketing campaigns. Develops and promotes community activities that work toward the objectives set forth in the Community Health Needs Assessment in collaboration with the Director and other Community Wellness team members. Provides education to the community at health fairs and local events. Assists with program planning and delivery for community events, initiatives, and wellness programs including CPR certification, MCH 10k Classic, MCH Health & Wellness Expo, and the annual Women's Conference. Participates in internal and external committees and resource groups to provide clinical expertise and provide representation for Community Wellness programs. Serves as an instructor for comprehensive diabetes classes. Assesses patients’ nutritional status according to established standards. Initiates outpatient record or obtains necessary information from the outpatient record to determine patient’s nutritional status. Provides nutritional teaching and counseling to patient/family in making desired changes in eating habits. Develops nutritional plan for patients of various ages and inclusive of cultural orientation of the patient. Communicates initial plan of care with referring physician. Evaluates the results of nutritional intervention of the patient’s nutritional status to determine the need to revise the nutritional plan of care. Consults with physician, Diabetes Education Program Coordinator and other members of multidisciplinary care team on appropriate nutritional interventions. Provides nutrition education & guidance to bariatric surgery patients through development, implementation and monitoring of dietary recommendations pertaining to weight loss surgery. Meets with bariatric surgery candidates multiple times during care plan to discuss nutritional needs pre and post bariatric surgery. Consults and coordinates with CFM, Health Information Management and Patient Registration departments to insure accurate registration, coding and processing to maximize reimbursement.
