ChristianaCare is searching for a Community Educator to promote access to services and provide health education while advocating, facilitating, and organizing access to services to improve the well-being of community members. This role serves individuals and families impacted by community violence.
To be successful in this role, the ideal candidate should have:
Experience with victims of community violence either personally or professionallyEager to learn and support various community programsNeeds to be flexible and open to changeHave the ability to travel between locationsPrincipal duties and responsibilities:
Creates connections between vulnerable populations, health care systems, and community/state resources.Assists patients in the community, home, and clinical setting. Establishes trusting relationships with patients and their families.Conducts intake interviews with patients and their families, while identifying socio-economic issues that impact their overall health. Develops health and social management plans and goals.Connects to internal and external resources using Unite Delaware platform.Documents patient engagement accurately and in a timely manner.Discusses cases in supervision with Community Health/Hospital Violence Intervention Program (HVIP) staff.Facilitates healthcare and social service system navigation.Establishes and maintains professional relationships with community leaders, faith leaders, and others responsible for providing services to the community.Participates in community events promoting Christiana Care Health System and Community Health services.Educates staff, health providers, and stakeholders about community needs.Provides culturally appropriate health education on health-related topics.Advocates for underserved individuals to receive appropriate services. Works to reduce cultural and socio-economic barriers between clients and institutions.Maintains professional competence by participating in local and national training sessions and conferences.Works collaboratively and effectively within a team.Establishes positive, supportive relationships with participants and provides feedback to other members of the team.Builds and maintains positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors, and office staff, from diverse cultural and socio-economic backgrounds.Participates in regular program and department meetings.Education and experience requirements:
