Sunday-Saturday (Shift Hours Vary)
Location: Bronx, NY
FLSA: Exempt
Salary: $62,800.00 - $62,800 (annual)
JOB DESCRIPTION
The role of the Community Habilitation Coordinator is to oversee and monitor the daily operation of the Community Habilitation services; documentation is complete and appropriate. In addition, he/she prepares billings; and completes all other activities assigned by the Director of Day Services, ensuring continuity and quality of Community Habilitation services.
RESPONSIBILITIES:
+ Conduct assessments of the functional skills of new consumers.
+ Develop, write, and oversee the Community Habilitation Service Plan implementation.
+ Assign caseload to Community Habilitation staff and initiate hiring new Community Habilitation Staff when indicated by enrollment numbers.
+ Supervise the Community Habilitation Staff.
+ Intervene on behalf of the consumer when the consumer, advocate, or other provider identifies a problem or presents a complaint.
+ Act as liaison between the Case Managers and the Community Habilitation Staff regarding the quality and appropriateness of Community Habilitation services and other consumer issues and needs.
+ Act as liaison between the Waiver Department and other DBIS Network departments, such as CCO, regularly or as needed.
+ Conduct regular reviews of consumer files to ensure all materials' currency, completeness, and appropriateness.
+ Conduct staff meetings, including training sessions.
+ Participating and attending meetings/training at various sites requires following different travel routes to the worksite.
+ Must have the ability to transport self to various work sites.
+ On-call 7 days a week, after hours, and on weekends to assist staff and families
Requirements:
+ Bi-lingual a must (Spanish & English speaking)
+ Bachelor's Degree in Health/Human Services or Psychology
+ Must have a Valid Driver's License

+ Strong leadership and organizational skills with attention to detail.
+ Ability to prioritize.
+ Flexible, poise, tactful, and able to develop a rapport with people from various backgrounds/positions.
+ Self-motivated with a pleasant attitude and the ability to work independently with the team.
+ Able to travel between work sites as needed.
+ Must be flexible, for hours can vary.
