Shift Worked: per diem Weekly Hours: 15 FLSA Status: Non Exempt Pay range: $17.00 - $20.00 The primary role of the Community Habilitation Specialist is to work alongside the people we support and provide Medicaid Waiver Services in community-based settings in order to help them to achieve their Personal Outcomes. This includes working with individuals with intellectual and developmental disabilities to develop daily living, professional, and decision-making skills while also supporting their health and safety. Position Duties: Provides training in accordance with valued outcomes specified by each specific Staff Action Plan and Life Plan. Provides training and supervision based on participant’s abilities including areas of self-care, independent living, community integration, work-related skills development, etc. Ensures the safety and security of the person receiving services by taking into account their specific needs and limitations. Direct services that support the participant(s) in the least restrictive manner. Ensures that each person is receiving services in the least restrictive manner, while working on their PCP goals to obtain greater independence Completes all documentation for service delivery in accordance with federal and state regulations. Provides timely services that are in accordance with each participant’s set schedule to ensure consistency and the respect of each person receiving services. Works collaboratively with the team that supports the person(s) receiving services. Responds appropriately to crisis/emergency situations in order to secure the safety and well-being of participant(s). Promotes program development by actively seeking participants in need of services. C. Requirements: + BA degree in human services or related field required. + Valid NYS Driver’s License, vehicle, and insurance coverage. + Minimum of 1 year working with individuals diagnosed with developmental and/or intellectual disabilities. + Good inter-personal, verbal-writing skills and be personal computer proficient with Microsoft Word. + Must be able to travel throughout Putnam and Southern Dutchess Counties if assigned. + Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
