Our hospitals, care sites and clinicians are recognized for clinical and operational excellence. **Job Description:** The Mercy Health- Lima, Community Health Coordinator will focus their effort on providing support on program development, data management, project management and outreach for all Community Health initiatives. This position requires someone who is self-motivated and can prioritize multiple and sometimes competing priorities. This role is part of a dynamic team who will need to work both independently and collaboratively with a broad spectrum of team and community members. This position will focus on both internal and external audiences, with the goals of; facilitating projects and programs that support community health goals in alignment to the CHIP (Community Health Improvement Plan) and other grant deliverables, working with cross-functional teams to enhance the pipeline of future employees in the healthcare field; making meaningful connections to thought leaders and influence in our service area, and supporting efforts and providing leadership to underrepresented communities in the form of programming and outreach. **Program Development** (20%): Partner with internal and external stakeholders to identify priority health needs, think critically about how to solve complex issues with the result of assisting in the creation of plans, setting goals, and identifying measurable outcomes for improved health outcomes for at-risk populations. **Data Management** (20%): Work with internal and external stakeholders to assist in the collection, analysis, and dissemination of relevant data as it relates to grant deliverables, CHNA/CHIP programming and additional opportunities around equity and disparity outreach. **Project Managements** (20%): Provide support in relation to on-going grant deliverables and the implementation of community health programming in partnership with internal and external partners. **Program Representation and Outreach** (20%): Represent Mercy Health at various community events throughout the year and manage on-going feedback and collections with external stakeholders in the forms of community conversations, advisory groups, or other communication opportunities further to build trust and foster positive relationships within the community. **Supervision** (10%): Provide supervision to Community Health Workers (CHW's) through the Pathways HUB program to review caseloads, attend trainings and serve as the liaison between Hospital Council of Northwest Ohio. **Pipeline Development** (10%): Provide oversight and guidance to pipeline programs which collectively serve underrepresented minority and/or students of under-privileged backgrounds from secondary school through post baccalaureate studies. **Required** : Bachelors in a health related field Many of our opportunities reward* your hard work with: Comprehensive, affordable medical, dental and vision plans Prescription drug coverage Flexible spending accounts Life insurance w/AD&D Employer contributions to retirement savings plan when eligible Paid time off Educational Assistance And much more *Benefits offerings vary according to employment status All applicants will receive consideration for employment without regard to race, color, national origin, religion, sex, sexual orientation, gender identity, age, genetic information, or protected veteran status, and will not be discriminated against on the basis of disability.
