Community Health Educator/Lifestyle Coach

POSITION PURPOSE

Facilitate community health education classes as indicated by the Community Health Needs Assessment.

REQUIRED EDUCATION, EXPERIENCE AND LICENSURE

Education: High school diploma or GED. Professional health care education, preferred. Education or training directly related to health program facilitation, patient/client engagement and behavior change preferred.

Experience: One to two years relevant community experience preferred. Active in relevant continuing education including maintaining DPP Lifestyle Coach training and other professional certification as appropriate.

Certification:  Certified Lifestyle Coach Certificate, as awarded by CDC, within 6-months of hire into position.
