\nThe Community Health Liaison works in community settings to gather data and/or deliver health promoting interventions following standardized protocols. Examples of data collection activities include conducting focus groups or interviews, gathering written surveys, extracting information from existing records, or direct observations. Examples of interventions include health education, skills training, coordinating health care services with community services, and coaching people or families to empower them to take control of their health and make the most of their healthcare.

\nESSENTIAL FUNCTIONS OF THE ROLE
\n\nBuilds relationships among community members, community groups, clinicians and researchers\nIdentifies mutual goals and potential barriers to collecting data and/or delivering interventions in healthcare and community-based settings as well as health needs of persons in the community\nCommunicates clearly about overall program goals and specific research or intervention tasks with community representatives, clinicians and researchers.\nPrepares materials and equipment (e.g., voice recorders) needed to gather data or deliver interventions according to protocols and predefined methodologies\nAdministers surveys and conducts focus groups and/or interviews by telephone or in person and documents data collection activities and participant responses accurately and according to protocols\nProtects the anonymity or confidentiality of data as required for the protection and respect of individuals and as stated in standardized protocols\nRuns health education activities, workshops, and classes and reports to all stakeholders about progress on planned tasks and any challenges implementing protocols as written.\nCommunicates concerns or problems encountered in activities/workshops/classes to supervisor in a timely manner to protect the wellbeing of participants and integrity of programs being delivered\nConducts skills training exercises in the community and coaches consumers in self care strategies and tasks for effectively engaging in healthcare based on standardized protocols\nReviews protocols and seeks feedback to ensure continued adherence to standardized protocols\n\nKEY SUCCESS FACTORS
\n\nExperience interacting with individuals or groups for the purposes of gathering information, educating or coaching. Examples of relevant experience include leading groups, interviewing individuals, conducting surveys, coaching teams, participating in health education classes, or other experience motivating healthy behaviors (e.g. acting as a trainer).\nCompleted or will complete as a requirement for continued employment training in the specific protocols required for data collection or intervention delivery tasks.\nAbility to explicitly communicate and model respect for stakeholders' unique perspectives and goals.\nAbility to accurately document and report on activities according to standardized protocols/operating procedures.\n\nBENEFITS
