Yes County Albany Street Address Chronic Disease Prevention, Bureau of Community Chronic Disease Prevention 150 Broadway, Riverview Center City Albany State NY Zip Code 12204 Duties Description The Community Health Program Manager 3 (CHPM3) will provide leadership and programmatic and administrative direction to a broad set of chronic disease programs in the Bureau of Community Chronic Disease Prevention (BCCDP). The CHPM3 will direct staff in a variety of activities including public health policy development, public health outreach with diverse communities, implementation of health care system interventions, linkages of community programs to clinical services, and coordination of programs with state, local and national public health partners. The CHPM3 will be responsible for managing a large and complex portfolio of federal funding used to leverage and augment existing State funding and is integral to the success of the statewide public health programs. Minimum Qualifications PROMOTION: One year or more of permanent competitive service as a Community Health Program Manager 2 (M3); OR TRANSFER: Current NYS employee with one year or more of contingent-permanent or permanent competitive service in a title at or above Grade 29/M3 and eligible for transfer under Section 52.6 of the Civil Service Law; OR NY HELPS: Bachelor's degree and seven years of professional* public health experience in a public health agency or program**. Two years of the experience must have been at a managerial level. *The required professional experience must include either: designing or implementing a public health outreach, promotion, or disease prevention program; OR conducting disease surveillance or a disease control program. ** A public health agency or program focuses primarily on the prevention of disease through outreach and public health education or health promotion activities, including population-based plans of care (not just plans of care for individual patients), or the study of the prevalence or causes of disease through population-based studies. Education substitution: A J.D. or master’s degree may substitute for one year of the general experience. A doctorate degree or Ph.D. may substitute for two years of the general experience. Examples of non-qualifying experience include, but are not limited to: providing clinical care in therapeutic health care facilities including public hospitals, physician's offices, supervised community-based settings or home care settings; discharge planning or utilization review; developing treatment plans for patients in community settings; regulatory oversight of health care providers to determine compliance with laws, rules or regulations; reviewing appropriateness of services for insurance companies, Medicare, Medicaid, governmental agencies or other third-party payors; or other experience limited to administrative support of public health programs such as operations management, personnel, staff development, fiscal administration, contract/grant management, budgeting or health care finance is not considered qualifying, nor is the provision of medical services to a fixed population. Preferred Qualifications: Experience in public health policy; experience working with various influencers to advance state and local policy; strategic planning; program planning; program evaluation; program monitoring; program management; grant administration; budgeting and fiscal management; staff leadership and supervision. Experience leading initiatives to improve health and health outcomes for marginalized communities, specifically communities of color. Experience actively soliciting feedback on public health interventions from minority and underserved communities. Additional Comments Permanent transfer or non-competitive NY HELPS appointment.
