Community Health Program Specialist 2
Job Summary Responsible for Community Health Needs Assessment and Implementation Strategy oversight and execution for ensuring completion of data analysis, writing and publishing of all Community Health Needs Assessments and Community Health Implementation plans and to comply with IRS 990 regulations. Must work with Corewell Health West team members and community partners to implement initiatives, develop data and initiative reporting, and deliver on programs within community. Work with Corewell Health, health equity evaluation and analytics team, finance and compliance experts on maximizing community benefit opportunities and local impact that leverages the time and talent of the greater Grand Rapids (Kent County) hospital personnel. Will attend and act as the Kent County Corewell Health CHNA liaison in all monthly health system CHNA meetings, hosted by the Health Department as well as internal Corewell CHNA planning and strategy meetings. Will support data methodology and evaluation best practices, working closely with the HEEAAT (Health Equity Evaluation Analytics and Assessment Team) to support rigor and enhancement to the existing CHNA process and product. This may include community based participatory research, leveraging existing community collaboratives and collective impact groups (Our Neighborhood our Health, A Just GR) among other methods and practices. Must build authentic and meaningful relationships with both internal Corewell and external community stakeholders. Will act as a liaison on coalitions and committees that are strategically aligned with the work. Individuals should have a strong project management approach with the ability to navigate competing priorities, timelines, and deliverables. Strong communication and presentation skills with the ability to communicate with a wide array of audiences and stakeholders. Essential Functions + Coordinates program planning, and implementation in area of accountability in collaboration with members of the health care team. + Develop and maintain network with internal and external partners to support programming. Develops support systems for client population and coordinates program with other system-wide and community support programs. + Oversees data collection and record management. Ensures that required documentation for activities within areas of responsibility is performed in a timely manner. + Recruits, and schedules program volunteers / staff. Maintains documentation of volunteer / staff activities. + Acts as a client advocate, crossing multidisciplinary lines to meet client needs. Communicates, collaborates, networks with and acts as a consultant to other members of the health care team within and outside Spectrum Health. + Identifies opportunities for philanthropic support to support program objectives. + Participates in Hospital / Community committees as member and consultant. + Demonstrates by attitude and action recognition of the importance of interaction with co-workers. Communicates effectively and respectfully with co-workers and staff of other affiliates.
