In this role, the successful candidate We have an exciting opportunity to join our team as a Per Diem Community Health Rep The New York University Grossman School of Medicines Center for Healthful Behavior Change housed in the Institute for Excellence in Health Equity is leading a new initiative that involves partnerships with several large health care centers including the Family Health Centers at NYULH and NYC Health and Hospitals Corporation The purpose of this project is to guide the implementation of a maternal health community implementation project We will engage diverse partners in learning hubs study implementation and best practices with the goal of improving maternal health outcomes and reducing maternal health disparities in the New York City catchment area The Per Diem Community Health Rep will assist in identifying and formalizing protocols scripts training materials and maintaining strategic partnerships This is an exciting opportunity to play a vital role in shaping and growing this initiative which is being implement across NYC
Job Responsibilities:
Program Development: nbsp;Assist in developing and managing training materials scripts PowerPoint presentations; Identify appropriate screening tools for social needs assessments; Contribute to the development of intervention tools and resources; Provide guidance and peer support to community health workers
Program Delivery: nbsp;Participate in group sessions and deliver educational materials; Conduct participant interviews and health assessments; Assist community members in navigating the healthcare system including scheduling appointments completing paperwork and accessing financial assistance program; nbsp;Assist with the informed consent process and ensure that the participant fully understands what is required of them throughout the study; Collect and maintain accurate records of client interactions program activities and health outcomes; nbsp;Mail study recruitment materials and other study related materials to participants; May coordinate and schedule group sessions
Strategic Partnerships: nbsp;Assist in building and maintaining partnerships with individuals families and community organizations; Communicate with clinicians community groups and other stakeholders; Assist in organizing stakeholder workshops and meetings maintain detailed meeting minutes and follow up on action items
Data Entry: nbsp;Input and update data into databases spreadsheets and other platforms; Verify accuracy and completeness of data entries reviewing information for errors or discrepancies and making corrections as needed; Conduct pre screening and troubleshoot participant enrollment using the patient facing dashboard; Collect participant information from electronic health record and other data sources for the research project
Other tasks: nbsp;Participate in staff meetings ongoing training and professional development opportunities; Participate in special projects and perform other duties as required; Contributes to preparation for conference and professional meetings

Minimum Qualifications:
To qualify you must have a High School Diploma and minimum of 1 year related experience in an office setting or the equivalent combination of education and experience is required Effective Oral written communication and interpersonal skills
Preferred Qualifications:
Associate s degree plus one year related experience or equivalent combination of education and experience Prior experience in a health care or community based settings
Qualified candidates must be able to effectively communicate with all levels of the organization.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $26.52 – $27.47 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
