This position will serve as an advocate for health plans members as well as a liaison between community, health and social service resources. They develop and maintain a relationship with health plan members and community partners to help alleviate the barriers and constraints members face in accessing needed healthcare and social resources. They provide culturally appropriate education and support. This position does not provide care to patients.

Responsibilities Interacts with health plan members and connects them with needed resources.Helps members with tasks such as scheduling appointments or assisting with completion of applications/enrollment for programs for which they may be eligible.Motivates members to be active, engaged participants in their health.Helps develop cultural awareness material and provides education and training to individuals, small groups and community partners.Advocates for health disparities at the individual and community levels.Knowledge / Skills / AbilitiesStrong interpersonal and communication skills.Able to work effectively with a wide range of constituencies throughout the community.Knowledge of community agencies and resources.Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.Ability to plan, implement, and evaluate individual member needs and goals.Knowledge of common, potential barriers to care.Ability to communicate medical information to health care professionals over the telephone.Skill in organizing resources and establishing priorities.Creative and analytical thinking. Qualifications QualificationsRequiredUtah Department of Health (UDOH) accepted CHW training certification or equivalent.Two years of experience providing community health worker services.Utah Driver's license and reliable transportation. Qualifications (Preferred) PreferredBilingual in English, Somali, Arabic and Spanish.At least two years in Social Work, Psychology, Public Health, or a related field.Working Conditions and Physical Demands
