Sentara Community Care Center Berkley is now hiring a Full-Time Team Lead, Community Health Worker in Norfolk, VA!

Hours: Mon-Tue (8a-5p), Wed-Thurs (8a-6p), Fri (8a-2p). Every other Saturday is required (8a-12p).

General Duties: The CHW Team Lead supports analytical/clinical work within the CHW program and serves as a subject matter expert for all CHW operations. Conducts SCOH screening for all customers, providing customers with additional resources, etc.

Qualifications:

Driver's License requiredHigh School or Equivalent requiredCommunity Health experience preferredSupervisory experience preferredHealthcare experience preferred
