GENERAL SUMMARY Provides administrative support to program case management. ESSENTIAL JOB FUNCTIONS* + Provides home visit relief to Nurses and Case Managers when needed. + Holds a small case load, and performances Case Manager role for that case load. Audits charts for completion. + Completes and updates identification sheets and photos. + Follows up via telephone to request missing caregiver logs/financial transaction records (FTR) and other essential case file information. + Tracks six month summaries and physician’s orders. + Tracks fire drills, home inspections, furnace inspections and safety plans. + Tracks caregiver trainings. + Files paperwork. + Tracks Individual Service Plans (ISP) and participant census via caregiver logs. + Tracks selected individuals’ financial information (RFMS). + Serves as key contact for Adult Foster Care determination information. + Completes paperwork for new program participants/caregivers. + Ensures adequate supply of commonly used forms. + Manages incoming and outgoing mail. + Maintains daily staff schedule. Updates the on-call list. + Orders supplies. + Maintains updated relief list. + Calculates caregiver respite balances and submits check requests to management for approval. + Provides information and referrals. + Serves as mandated reporter for suspected abuse and/or neglect of individuals to the Disabled Persons Protection Commission (DPPC). PREPARATION, KNOWLEDGE, SKILLS & ABILITIES + High school diploma. + Valid driver’s license in the state of Massachusetts. + PC proficient. + Ability to work under pressure. + Ability to multi-task. SUPERVISORY RESPONSIBILITY + No supervisory responsibility. WORKING CONDITIONS/PHYSICAL DEMANDS + Ability to lift a minimum of 20 pounds in office supplies.
