Community Health Worker

Find your purpose with CentraCare working as a Community Health Worker in Melrose! The Community Health Worker is a member of the community who applies their unique understanding of experience, language, and/or culture to communicate a comprehensive health education program to diverse members of the community specifically in a Spanish education partnership with parents, families and children to complete behavioral-developmental screening.

The Community Health Worker will be responsible for assisting patients with a variety of care coordination activities. Activities will vary based on patient need, but basic functions will include helping patients to navigate the healthcare system, understand and be connected to CentraCare Clinic Women & Children's to eliminate barriers to care.

Schedule Full-time | 64 hours every two weeks | Melrose Community Monday - Friday | 8:00 am – 5:00 pm | Rotating day off each week  No weekends | No holidays

  Qualifications  Associate’s degree or higher  Spanish language interpretation REQUIRED  Experience in community outreach preferably in Spanish community and working knowledge of various systems of health care  Community Health Worker Certification required within 18 months of hire  Current American Heart Association Basic Life Support (BLS) card prior to completion of unit orientation Valid Minnesota driver’s license, a driving record in good standing and the minimum amount of insurance liability coverage accepted per CentraCare policy Candidate will have reliable transportation and be driving their own personal vehicle for this position
