The Community Health Worker (and PD) provides overall support for the CARE Network clinical field staff in the delivery of case management services. Under the direction of the RN and Social Work team, provides case management support to program clients in the community/home setting, as well as assistance and support with office clerical functions as needed. CARE Network provides community based, comprehensive, disease management/case management services for the chronically ill, which may include: Heart Failure, Cancer, HIV/AIDS and other chronic diseases.

Required Qualifications:
upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. 1 year Experience in a public or private social services agency.Preferred Qualifications:
