The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y.
Jordan Health is now seeking a Community Health Worker who, under the guidance of the Director of Social Emotional and Mental Health, is primarily responsible for providing outreach, follow up and referral in a medical setting. The Community Health Worker works collaboratively with others in order to make critical linkages to health information and community services for patients.

RequirementsThe Community Health Worker will ensure to:
· Consistently engage in fulfilling the various social determinants of health needs for Jordan patients.
· Promotes professional growth as a Community Health Worker.
· Increase accessibility of healthcare for patients.
· Increase community outreach.
· Consistently works collaboratively with internal teams and external teams to support patient care.
Education And Experience Required:
· Associate's degree in human services or commiserate experience in outreach/community health.
· Medical office/hospital or outreach experience a plus.
· Experience with diverse cultures and populations preferred.
Special Skills, Knowledge Required:
· Must have a valid driver’s license and reliable transportation.
· Bilingual language skills preferred.
· Office or medical hospital experience a plus.
