Community Health Worker

Job Description:

The Community Health Worker (CHW) has accountability to partner with clinical and health plan teams to engage, advocate, educate, navigate, and coordinate with and on behalf of individuals who have been identified as having social needs that impact their health. The CHW will primarily be an embedded resource within the community and will able to visit patients and members in home or community settings while also partnering with care management and care teams (e.g., clinics, emergency departments, homecare, health plan, etc.). This Community Health Worker will work under the direction of a Care Manager and/or clinic RN/MA.
Our Community Health Workers (CHWs) are based at the Nevada Corporate Office but are dedicated to serving the community. They are deployed to various locations, including our primary care clinic, inpatient or post-acute facilities, and even home visits, depending on the needs and support required by our patients.

Core skills and roles include:

Establish trusting relationships with patients and their families to help them become more engaged in their health. Help patients identify their potential; work to increase individual empowerment and create community connections.Conduct intake interviews with patients utilizing motivational interviewing and health coaching techniques.Facilitate timely enrollment and/or referral into applicable health and community resources. Assist the patient in navigating to referrals and health-related care settings, if needed.Follow-up with patients via phone, home visits, and visits to other community settings.Work closely with care managers and clinical teams to ensure patients have comprehensive and coordinated care. Contribute, in collaboration with Care Management, to the social needs areas of the patient’s plan of care.Support community partnerships in coordination with Community Health. Maintain relationships with key stakeholders in the community by serving as an advocate to improve health and social outcomes.Effectively communicate with patients in a manner that is easily comprehended and conveys empathy while also respecting the desires of the patient and their family.Contribute to the knowledge base of applicable community resources and their intended use or purpose.Participate in professional development and networking opportunities.Willing to become certified as a community health worker within the first year to 18 months of employment.

Minimum Qualifications

Fluency in English. Also fluent in Spanish/or another core language in targeted communitiesDemonstrated communication skills (e.g., listening, speaking, writing)Demonstrated ability to work independently and in groups. Ability to take initiative and be proactive.Able to conduct home visits. Willingness to be present on-site in clinical and community spaces.Basic computer skillsCurrent driver’s license, insurance, reliable transportation, and an acceptable driving recordWilling to become certified as a Community Health Worker

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
