Position Summary

The Community Health Worker is part of an interdisciplinary care team that assists in care coordination, informing patients on community resources, and improving population health.ResponsibilitiesPerforms cross continuum care activities, which may include coordinating care between primary care clinics, specialty care clinics, urgent care clinics, or other sites of care, as appropriate.Engages in outbound communication to patients and keep them engaged in their care, set-up appointments, and prompt them to act.Helps patients access community resources which may include scheduling appointments.Establishes contact with community partners, travel is required.Develop tools to educate peers on patient advocacy and community resources.Participates in community health fairs and events.Educates the care team on available community resources.Participates in quality improvement activities.

Requirements

High School Diploma or equivalent2 years of health care related experienceCurrent Washington State Driver’s License; and proof of a safe driving record (which meets established MHS Standards) obtained by a motor vehicle report from the appropriate state.Experience in utilization management, community resources, and/or financial and quality issues affecting patient care, preferredCommunity Health Worker experience, preferred

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $24.33 - $35.01 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
