Community Health Worker

We recognize that our diverse workforce allows us to provide culturally competent care to everyone in our community. The Community Health Worker (CW) provides a bridge between providers and patients. The Community Health Worker will provide psychosocial support services to individuals living with HIV/AIDS, including risk reduction, basic HIV/AIDS education, and medical adherence strategies through various programs (including the HOPWA State,MRT, YouPLUS, and PAWH 50PLUS program) which provide key education to patients living with HIV/AIDS about HIV transmission and how to reduce the risk of HIV transmission. As part of the essential functions of this role, the Community Health Worker : + Understanding of issues related to living with HIV/AIDS + Familiarity with HIV/AIDS services in the community + Provides good oral and written communication skills + Assist team with outreach, contacting and engaging patients, accompany patients on medical and supportive appointments + Assist Health Educator with group and individual sessions that focus on strategies for achieving positive health outcomes + Assists patients in identifying risk reduction strategies + Assists with outreach opportunities, including face-to-face meetings, tabling events, and other goals and objectives for reaching the target population + Works with participants to link them to care coordination and HIV medical care; assists participants with appointment reminders when needed; accompanies participants to appointments if needed + Reaches out to patients who are not engaging with the Case Managers in order to re-engage them in care. This outreach could include activities such as sending letters, conducting home visits, or meeting patients at a medical appointment + Documents all outreach and linkage activities on program forms and in Medent according to program procedures and within required time frames; completes all required reports + Tracks progress in accomplishing work plan goals; identifies barriers where goals are not being met and communicates them to the Director + Collaborate and support the team members to ensure a patient-focused approach to care + Supports health educators with the development and implementation of health education sessions; can serve as a co-facilitator Qualified candidate will have a high school diploma and at least two years of related experience; candidate must have strong written, verbal, organizational, customer service, and time management skills.
