Job Description
This role focuses on improving the well-being of individuals, families, and communities by addressing medical, behavioral, and social determinants of health. It requires a combination of healthcare knowledge, problem-solving skills, and the ability to work collaboratively with members and healthcare providers.
ResponsibilitiesConduct Assessments of Member Healthcare Needs: Engage members in discussions about their comprehensive assessments including bio-psychosocial, functional, and behavioral health needs. Identify and escalate urgent member situations.Recommendations for Member Care and Service: Collaborate with members to develop healthcare goals and identify appropriate programs. Utilize motivational interviewing techniques.Assist With/Recommendations to Improve Member Health: Coordinate member access to health-related programs and services. Monitor progress and make changes to plans based on outcomes.Educate Members/Caregivers in Self-Management and Plan Benefits: Provide education on preventative healthcare measures and plan benefits. Educate on disease-specific signs and symptoms. Promote member health and wellness.Use Technology to Obtain and Document Health-Related Information: Research and document member-related health information. Use computer applications to summarize and communicate information. Navigate multiple applications to obtain and document member information.Additional Skills & QualificationsMinimum: High school education or equivalent experiencePreferred: Bachelor's degree in social work, licensed practical nurse (LPN), CNA/HHA, or 3 years of experience in a community health settingPay and Benefits
The pay range for this position is $20.00 - $22.00/hr.
