Community Health Worker CHW works to promote maintain and improve the health of underserved residents and their families by providing social support and basic information to promote healthy behaviors This position uses their cultural connectedness to the community to provide information advice and support related to healthy behaviors and access to health and community services This position works as a member of the care team through an integrated approach to care management and community outreach CHWs are connected to the community they serve and are recognized as a ldquocritical linkrdquo to connect underserved residents to beneficial resources and navigate the ever changing healthcare environment Specific activities of a CHW include screening clients in the community for Social Determinants of Health SDOH which includes housing food security transportation utilities personal safety education childcare and financial strain and then taking action to address those needs Job Responsibilitiesmiddot Establish trusting relationships with and act as a liaison for clients to access resources and agencies that assist the client to better manage their healthmiddot Screen residents for the Social Determinants of Health SDOH and then provide clients with the information needed to begin taking action to address those needsmiddot Provide information to clients about the benefits of preventative care to help avoid andor address health conditions including but not limited to blood pressure monitoring and blood sugar monitoring Motivate clients to be proactive about their health while providing general support and encouragementmiddot Provide follow up appointments and referrals at community health screenings for clients in need of connection to a primary care physician or in need of follow up for an abnormal screeningmiddot Assist with data collection and data entry for community screenings community surveys etcmiddot Attend health fairs and other community events to provide information on effective methods of disease prevention and to share resources available in the Hartford areamiddot Travel between various locations within the community to SpanishAbility to speak and understand the language of the community being served preferredMust be proficient in a variety of computer software applications Microsoft Word Excel and other Office applications as necessary Ability to quickly build strong relationships with targeted community residentsAbility to organize prioritize and planschedule work tasks Good verbal and written communication skillsAbility to exercise good judgment related to recommending servicesMust possess the ability to handle sensitive and confidential situations Long term resident of Hartford area strongly preferredComfortable with working directly in the community and in non traditional settingsMust be able to travel between various locations within the community to perform duties ndash valid
