Hartford HealthCare network and is the systemrsquos second largest hospitalProject OverviewFor this project incumbent will be responsible for work related to two grants There will be a focus on Breast Health encouraging community members to receive screening mammography and promoting breast health In addition incumbent will focus on identifying health related social needs and work to meet those needs through available resources and community connections The incumbent will serve as part of the Mission ServicesCommunity Health team and will be involved in many community health related programsThe community health worker willConnect patientscommunity members and families to resources and services that address their social influencers of healthProvide referrals and connection to serviceParticipate in bi weekly food distributionAttend health fairs and other community based health promotion eventsParticipate in Medical Mission programAssist with CHW caseload as needed to ensure
