Job Highlights:

This is a full time day shift grant funded Community Health Worker position working 8 hour shifts Monday through Friday. This is a great opportunity for someone with an interest in public health. The main duties include working to understand patient barriers/needs and connecting them with community resources.

Pay Rate: 18.69 per hour

Schedule: 40 hours, M-F, 8 hour day shift. Start time is flexible but no later than 9:00 a.m.

Job Summary:

Community Health Workers primarily work out in the community with community members of diverse backgrounds. This role works closely with healthcare teams and other community-based organizations through activities related to health priorities identified in the Community Health Needs Assessment and Improvement Plan.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

Bridge the gap between communities and assist community members in navigating the health and social services systems and establish positive, supportive relationships with community members and provide feedback.Promote wellness by providing culturally appropriate health information and education to clients and providers. Assist clients in accessing health related services, including but not limited to obtaining a primary care provider and overcoming barriers to obtaining needed care and social services.Increase self-efficacy to achieve wellness and help community members in accessing resources, including scheduling appointments, and assisting with completion of application for programs for which they may be eligible. Provide direct services including lifestyle and survey assessments, health education, and basic health screenings and document all activities as detailed by direct supervisors. Leverage strong interpersonal and communication skills while effectively working with a wide range of constituencies in a diverse community.Work collaboratively and effectively with a team, build and maintain positive working relationships with the community members, healthcare teams, and community-based organizations. Continuously expand knowledge and understanding of community resources, services and programs.

EDUCATION

High School diploma/GED or 10 years of work experience

EXPERIENCE

One year experience working with people from diverse backgrounds or community programs

PHYSICAL REQUIREMENTS

Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.Frequent keyboard use/data entry.Occasional bending, stooping, kneeling, squatting, twisting and gripping.Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.Rare climbing.
