This position will support the GRASSROOTS Health project within the Department of Management, Policy, and Community Health. In this position, you will provide outreach and care navigation for low-income and formerly homeless adults with Hepatitis B or C.

Position Summary:

Seeking a part-time (20 hours per week) Community Health Worker to work on the project providing viral hepatitis B and C education, screening, vaccination, and treatment to individuals with experiences of or at risk for homelessness. The work will be conducted off-campus at housing sites in the San Antonio area. The incumbent will be asked to visit housing sites weekly, assist other staff, help with care navigation for hepatitis B and C, help with data collection, and prepare reports.   

 Position Key Accountabilities:

Attendance at viral hepatitis screening events Provide education on hepatitis B and C (training will be provided) Meet with individuals interested in testing, vaccination or treatment Record all job activities in a data management system Support the phlebotomy team in paperwork, laboratory tests ordering and blood samples drop off  Contact individuals to inform them of viral hepatitis status Contact individuals to discuss vaccination and treatment options Navigation of clients to clinical care Secondary Responsibilities (as necessary if time allows): Contact individuals for vaccination follow-up Attend weekly team meeting other related duties as assigned.

Certification/Skills:

Texas Department of State Health Services (DSHS) – Certified CHW Bilingual (English/Spanish) preferred Must have ability to perform complex and diverse administrative duties that involve application of procedures and demonstrate appropriate judgment within the CHW scope of work.

Minimum Education:

High school diploma or GED.

Minimum Experience:

One (1) year of relevant work experience in health, social worker or education.  May substitute required experience with equivalent years of education beyond the minimum education requirement.
 

Physical Requirements:

Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly to move objects.
