What we want
The Community Health Worker II recruits participants, assesses community needs, and connects participants to social and community support resources. This position co-leads data collection and program assessment and provides periodic support to Together for a Better Tomorrow program functions.

Salary: Compensation will be commensurate to the selected hire’s experience.

Other Factors/Requirements: Persons in this position should be able to work beyond normal office hours including adaptable work hours on weekends/evenings. This position will require occasional travel outside of the Lower Rio Grande Valley as well as substantial travel within the Lower Rio Grande Valley (multiple Counties). Persons in this position should be able to travel independently to other and/or remote locations by driving and/or flying on commercial airlines. This position will require the ability to temporarily store data in home residence.

Qualifications
Required Education and Experience
+ Associate degree in applicable field or equivalent combination of education and experience
+ Four years of related experience
Required Licenses and Certifications
+ Department of State Health Services (DSHS) certification, certified CHW and/or CHWI, Promotora Certification of Training, or ability to successfully complete Promotora Certification
Preferred Qualifications
+ Two years of related experience as a Community Health Worker working with Spanish-speaking community members, stakeholders, and completing research and outreach activities
+ Public relations and marketing experience
+ Experience with assisting or leading training efforts
+ Experience with data collection and data management efforts
+ Experience with community-based participatory research efforts
Knowledge, Skills, and Abilities
+ Strong verbal and written communication skills in English and Spanish
+ Ability to multitask and work cooperatively with others
+ Ability to work with sensitive information and maintain confidentiality
+ Strong interpersonal and organizational skills
+ Knowledge of word processing, spreadsheet, and database applications
+ Strong computer literacy including ability to troubleshoot and provide technological support
Responsibilities
Program Planning & Implementation: Participates in participant recruitment, program planning, implementation, and evaluation including managing appointments, developing program material, assisting support staff in the flow of participants, and data collection. Participates in research related to health promotion and disease prevention. Serves as an instructor to CHW students and trainees in formal, structured programs.
Data Management: Manages program data to meet program and participant needs, including coordination and communication of program records. Coordinates appointments, ensuring that appropriate medical records are available.
Program Communication: Communicates with “Together for a Better Tomorrow” participants, serving as a liaison between program research staff, community members, and stakeholders. Assists patients seeking financial and other social and/or community support services.
Other Duties as Assigned: Performs other duties as assigned.
