In this role, you will serve as a crucial liaison between healthcare providers and community resources, providing education, advocacy, and support to improve health outcomes and reduce barriers to care. You’ll work directly with patients and community members to facilitate access to services, increase health knowledge, and empower individuals to manage their well-being.

Key Responsibilities:

Facilitate communication and patient empowerment during interactions with healthcare and social service systems.

Troubleshoot barriers patients may face in adhering to care plans.

Engage with the community through outreach to recruit and support high-risk patients.

Serve as a link between healthcare/social services and the community, ensuring access to culturally competent services.

Assist the primary care team with care management processes, such as patient education and chronic disease self-management.

Conduct community education activities to build knowledge, self-sufficiency, and access to resources.

Collaborate with patients and their care teams to address barriers to achieving healthcare goals.

Connect patients with appropriate community resources and health center services.

Follow up with patients through phone calls, home visits, and community outreach.

Conduct public education and provide fair, impartial information about qualified health plans (QHPs) and facilitate enrollment in these plans.

Perform general support activities and carry out special projects as assigned.
 

Education and/or Experience Qualifications:

High School Diploma or equivalent required.

Bachelor’s degree in a related field from an accredited college or university preferred.

Two (2) years of experience in community health, human services outreach, or healthcare preferred.

Desirable experience includes customer service, patient/peer coaching, medical patient interaction, community relations, outreach, data entry, and public speaking.
 

Additional Qualifications:

Knowledge of harm reduction, motivational interviewing, and other relevant treatment modalities.

Familiarity with local community culture and values.

Strong understanding of customer service principles, including needs assessment and satisfaction evaluation.

Excellent verbal and written communication skills, including active listening and clear information delivery.

Proficiency in Microsoft Excel.
