Job Summary
The Community Health Worker Lead executes duties with excellence by supporting and supplementing Novant Health’s Care Teams. These duties focus specifically on wide range of determinants for the health outcomes of diverse populations, with intentionality for historically underrepresented patients to bridge gaps between communities and health and social service systems. This team member provides and helps to access culturally and linguistically appropriate health education, information, and outreach in both healthcare facilities and community-based settings – in homes, clinics, shelters, etc. Collaborates with members of a patient’s healthcare team to assess needs, develop, implement, and review care plans to assist patients in receiving the optimal level of care at the most appropriate place of service. This includes conducting health screenings, providing health education/coaching, and linking patients to appropriate community resources. The CHW III maintains expert knowledge of available community resources and community outreach program options for referral.
The Lead title is so designated because of additional leadership duties this person takes within the team. This person will assist in onboarding new CHW team members, both at the CHW I and CHW II levels, and will arrange shadowing opportunities with other teammates as well as with other clinical services within the hospital and nonprofit agencies. This person will play a vital role in establishing Novant Health, neighborhood, and non-profit contacts in their market.
The Community Health Worker Lead will become part of the leadership team and assist in coaching, mentoring, and teaching other CHWs in their market, and this person will have a defined voice in strategizing on the future of the program. The Community Health Worker III will represent the team to internal and external partners, will demonstrate best practices, and provide motivation and define and maintain accountability standards. This person will have broad and accessible knowledge of community resources and will serve as a peer-to-peer source for problem-solving for teammates in the field.
The Community Health Worker Lead will have an adjusted caseload size determined in partnership with Director to allow capacity to fulfill their leadership duties.

Qualifications
+ Education: High School Diploma or GED, required. Some college, preferred.
+ Experience: Minimum of two-year experience in community setting and linking patients to various community resources, and at least one year experience as a Community Health Worker; required.
+ Licensure/Certification:
+ Health Coach Certification, preferred.
+ NC CHW Certification, required.
+ Additional Skills Required: Member of the community with understanding of the population he/she serves. Must be able to function as part of, and a supportive and effective leader of, a team. Ability to travel and work a variable schedule including some nights and weekends. Strong organization, team building, communication and leadership skills. Ability to work independently without supervision and self-manage. Ability to effectively communicate with a wide range of individuals and constituencies among a diverse population. Have a passion for health improvement and be able to communicate case sensitive material to participants. Accepts and respects diversity without judgement. Ability to respect confidential information. Ability to model best practices relative to deadlines, patient privacy, team member safety, documentation, and partner relationship building. General working knowledge of motivational interviewing, behavior change methods, and an emphasis on preventive care. Must be able to work with changing priorities. Experience with Electronic Medical Records, Word, Excel, and Power Point software.
