ChristianaCare’s Office of Health Equity and the Helen F. Graham Cancer Center & Research Institute (HFGCCRI) are searching for a Community Health Worker (CHW) to engage residents in Wilmington communities previously identified as having lower rates of breast cancer screening and elevated rates of advanced breast cancer.
To reduce the overall burden of breast cancer and close disparities, the CHW will engage residents from prioritized communities to provide education on breast cancer prevention, improve access to breast cancer screening (mammography) and risk assessments, and serve as a liaison to the healthcare system.
Principal Duties and Responsibilities Include:Attend meetings with members from the Delaware Division of Public Health, HFGCCRI, and Office of Health Equity to complete necessary CHW and other educational trainings.
Assists individuals in the community and/or clinical setting to get connected to breast cancer screenings, necessary assessments, and referrals.
Participates in the development of breast cancer health education and training programs and delivers them in the community.
Creates health system and community connections for individuals in vulnerable populations, especially for those with elevated breast cancer risks.
Establish connections with community leaders and groups who provide services to desired communities.
Education, Experience, and Role Requirements:High school diploma required; bachelor’s degree preferred.
A minimum of one year of community and patient-facing experience required with diverse populations, including volunteering at local community centers, healthcare experience, or previous experience as a CHW or Community Educator.
Strong written and verbal communication skills, previous experience leading educational workshops preferred.
Bi-lingual language skills (Spanish and English) are preferred.
Completion of a recognized Community Health Worker Core Competency Training program or willingness to complete upon hire.
Ability to reliably travel by vehicle and work flexible schedule, including some nights/weekends for necessary work events or conferences.

Candidates will be appointed for one-year term, with full salary and benefits. This position is grant funded by state funding; continuation of this position is contingent upon the program being renewed.
