**Employment Type:** Full time **Shift:** **Description:** The Linkage & Liaison CHWB Coordinator assists with increasing access, communication, community engagement and educating of case managers providing services to HIV patients throughout Broward County. This position assists with educating, linking, and providing guidance to access care and treatment and remaining in care. The selected candidate will act as a liaison between Holy Cross Health and the community, and must have excellent customer service skills along with experience collecting data, using electronic databases and other computer software packages. The selected candidate will be required to have a personal vehicle available for use with mileage reimbursement. The candidate will also be required to possess a motor vehicle operator’s license valid in the State of Florida. The candidate will be required to work day shift, evening shift and weekends as needed. Individuals fluent in Spanish, Creole, and English are strongly encouraged to apply.
