Community Living Skills Coach Rochester, NY (http://maps.google.com/maps?q=67-89+Canal+St.+Rochester+NY+USA+14608) Job Type Full-time Description Responsibilities: The Community Living Skills Coach will provide education, training, and support to residential clients, empowering them to maintain the highest level of independent community living. Reporting to the Director of Residential Services, the incumbent will work collaboratively with clients, staff, and external partners to achieve program goals. Results Measures: 1. Services That Address Client Goals: + Partner with clients to pursue Service Plan goals through coaching, teaching, and counseling. + Collaborate with East House staff, RPC, and other housing providers for coordinated client services. + Manage caseload efficiently, ensuring accessibility to each client. + Address client needs during on-duty shifts. + Coordinate with designee to assist clients with funding and financial matters. Results: + Clients make reasonable progress toward goals. + Positive satisfaction feedback from colleagues. + Appropriate response to resident needs during on-duty shifts. 2. Health and Safety: + Teach, support, and assist in the daily upkeep of the environment. + Support clients in cleaning, meal preparation, and maintaining safe and sanitary conditions. + Assist clients with appointments and treatment recommendations. Results: + Maintenance of food/meals during on-duty shifts. + Consistent coaching for cleaning and meal preparation. + Living environment consistently safe and sanitary. 3. Safely Managed Medications: + Provide supervision for clients managing medications. + Adhere to East House medication policies and procedures. Results: + Medication documentation and supervision meet East House standards 100% of the time. + No medication errors. 4. Documentation Standards Met: + Maintain documentation to the standards defined by regulators, funders, and East House policies. Results: + Documentation standards met as evidenced by audits and reviews. Requirements Education/Experience: + High School + 1-2 yrs. Exp; OR, + Associates + 1 yrs. Exp; OR, + Bachelors + 0-1 yr. Exp Autonomy: + Performs day-to-day duties independently, typically needs additional feedback or support when issues arise. Complex issues typically require additional support. Interpersonal Interaction: + Works well with clients and employees. Helps others as necessary to accomplish Agency goals. Agency Involvement: + Involved in a limited number of agency committees, focus groups, teams, etc. Licensure/Certifications: + NYS motor vehicle license, safe driving record, and availability of a personal vehicle for work. + Qualified Health Professional as defined by OMH Residential regulations or OASAS Operating Regulations preferred. Other Skills/Knowledge/Experience: + Effective written and oral communication skills. + Ability to work cooperatively with culturally diverse clients, staff, and community service providers. + Basic Computer Literacy, including basic Microsoft Word skills. Physical Demands: + Job involves sitting, handling objects, working with a computer, and communication. + Occasional lifting of up to 30 lbs., walking, standing, and reaching. + Finger dexterity, hearing, and visual requirements. Working Conditions: + Respond to crises and emergencies during on-duty hours. + Availability for work during evenings, weekends, and holidays. + Position requires travel across a six-county region and occasional transportation of clients. Salary Description $17.65-$26.55
