The Community Outreach Manager is responsible for expanding Episcopal SeniorLife Communities’ Neighborhood Program to Hilton, Gates, Chili, and other areas located west of the City of Rochester. The Community Outreach Manager will build new community partnerships and neighborhood programs that will improve and enhance the health and well-being of the older adults living in the community based on the needs and wants they have identified. This position will work with the Director of Dementia Services to ensure all neighborhood program expansion integrates with programs and services specifically designed to serve individuals affected by Alzheimer’s and other dementias and those at greater risk for Alzheimer’s and their caregivers by ensuring access to ESLC’s high quality dementia care and support. The Community Outreach Manager will ensure all neighborhood programs’ standards, processes and procedures align across all ESLC neighborhood programs. In collaboration with the Supportive Housing Manager and Supportive Housing team deliver dementia friendly health and wellness programming to an assigned region, coordinate care for individuals living in supportive housing under the Empire State Supportive Housing Initiative (ESSHI) at St. Leo’s in Hilton and ensure that care navigation is available to people living with dementia and their caregivers under the CMS Guiding an Improved Dementia Experience (GUIDE) Model.


 Responsibilities for a Episcopal Seniorlife Communities Community Outreach Manager

Develop and deliver dementia-friendly health and wellness programming and a robust volunteer program Ensure that all programming is dementia friendly and meets the highest standards of quality care and customer service. Specifics activities include but are not limited to: Building new relationships: developing and maintaining positive relationships with community membersFacilitating collaboration: driving internal and external collaboration to expand the offerings of the neighborhood programAttracting new members and driving increased participation: promoting and encouraging participation in the program Expanding and delivering health and wellness programs in partnership with a volunteer workforce Promoting the program through a tailored balance plan, nutrition education, strength training, informative presentations and other activities Ensuring that all programs have a measurable impact on the participants by delivering qualitative and quantitative assessments and surveysAll programs are inclusive for all communities that reside within each of the neighborhoods. Developing and delivering educational programs that address all aspects of healthy living through seminars, lunch and learns, trips to community events and programs, (such as art, music, and socialization opportunities)Recruit, supervise, and coach a team of volunteers to deliver programs in a culturally competent manner. Ensure accountability by regularly monitoring volunteer impact and actively taking action to improve volunteer performance. Represent ESLC at networking, recruiting, and tabling events in collaboration with the ESLC outreach team.Complete all health and quality care assessments as required by funders. Make adjustments to the program to ensure high participant satisfaction and impact. Track and report necessary information/data to fulfill any grant requirements. 

Care Navigation 

Support and closely coordinate all care needs including access to clinical and community support services for residents of St. Leo’s supportive housing program as well as GUIDE beneficiaries.Execute quality of life and home safety assessment screening tools to beneficiaries to assess home safety, behavior, function, medication monitoring & support needs, care needs, and advanced care plans.Assess and document caregiver concerns including caregiver strain, depression, and poor coping.Develop personalized care plans based on the desires and needs of each beneficiary and their unpaid caregivers based on the results of the completed assessments.  Review and update care plans as the participant’s condition evolves and as GUIDE and ESSHI requires. Complete required written documentation in the electronic health record and other required reporting systems on a timely basis as it relates to in-person, virtual, and telephone encounters. Complete all follow up activities as required for a comprehensive care plan that ensures the ability of the beneficiary to remain in their homes and communities. Develop and deliver educational and support programs to beneficiaries and their unpaid caregivers as required by GUIDE and as dictates the best interests of the dyad (person living with dementia and their unpaid caregiver).Provide guidance on home safety, communication techniques, responding to challenging behaviors, and managing caregiver stress to help people living with dementia remain in their homes as long as possible. Comply with all regulatory requirements.Escalate beneficiary concerns to the clinical team to maintain an appropriate scope of practice for this role.Abide by HIPAA regulations to ensure beneficiary privacy.Refer to ESLC’s 24/7 helpline service for any immediate support and advice. Understand and meet requirements of the Empire State Housing Authority’s supportive housing program.Ensure participants’ service plans and health and wellness assessments are current as per Department of Health guidelines. Work with outside vendors to coordinate services based on the needs of the Supportive Housing participants. Perform other necessary duties as required. 


Requirements

QUALIFICATIONS AND EDUCATION:

Bachelor’s degree in wellbeing related fields, such as health and fitness, community development, recreation therapy, social work, nursing, or physical or occupational therapy, or Associates’ degree with 3-5 years of experience in building community relationships or equivalent related work experience. Detail-oriented and thorough: the ability to independently manage the details of work and complete tasks thoroughly.A dedicated, energetic, and passionate individual with a strong work ethic and desire to grow professionally.Able to identify and implement best practices to enhance quality of life for people living with dementia and their caregivers.Ability to organize your day and work independently as well as be flexible and work in a team environment.Identify and cultivate relationships with community partners, organizations, volunteers and diverse populations to build a person-centered care plan that responds to an individual’s needs and preferences and contribute to achieving the individual’s goals.Demonstrates a strong sense of ownership and initiative in tackling new challenges, consistently striving to exceed goals and deliver exceptional results with minimal supervision. Excellent communication skills.Knowledge and experience with Alzheimer’s and other dementias; keep informed of the latest developments, progress and research updates in Alzheimer’s disease and related dementias.Ability to work with people living with dementia and their caregivers with empathy and professionalism.Valid New York State Driver’s License required.Reliable transportation.

Performing duties of this job require occasional walking and standing in and around the residences and community. Must be able to occasionally lift loads of 30 pounds without assistance and the ability to sit, talk, and hear is required. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, color vision, and the ability to adjust focus. This position requires extensive travel.

Takes an active role in all fire drills during the shift and/or emergencies for internal and external disasters as monitored by supervisor and incident manager.
