Community Outreach Patient Navigator

As a Community Outreach Patient Navigator, you will serve the community by decreasing disparities through navigation and education. Caregivers in this role build working relationships, solve problems, remove barriers to care and support patients while assisting them through the continuum of care. Community Outreach Patient Navigators also educate people within the community on diseases, prevention and health screenings while identifying and linking eligible patients and their families to required screenings and services and coordinating appointments and testing for early detection of medical issues. Opportunity for advancement and promotion is expected in 2-3 years.
**A caregiver in this position works 8:00am - 5:00pm + calls.**
A caregiver who excels in this role will:
+ Coordinate and educate patients within the community on disease prevention and screening.
+ Identify and address barriers to care, fear, trust, financial, insurance, transportation and psychosocial to ensure appropriate clinical care is coordinated and provided in a timely manner
+ Help patients navigate our complex health care systems.
+ Coordinate appointments related to health screenings, such as mammogram results, colonoscopy results and pathology reports.
+ Navigate patients to appropriate health care providers within Cleveland area facilities.
+ Identify barriers to care and coordinate resolution to social work support, Medicaid, Medicare, prescription assistance programs, community resources, primary care services and medical homes.
+ Support the coordination and execution of community events.
Minimum qualifications for the ideal future caregiver include:
+ High school diploma or GED.
+ Three years of related experience in customer relations working with diverse patient/culture populations.
Preferred qualifications for the ideal future caregiver include:
+ Spanish speaking.
+ Epic or other electronic medical record (EMR) experience.
+ Skills/qualities: team-oriented, communication, organization, interpersonal, problem solving, analytical and critical thinking

**Physical Requirements:**
+ May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating. May need to remain still for extended periods, including sitting and standing.
+ May need to lift boxes or carry objects
+ Working evenings and weekends is required
