Job SummaryThis role is designed to serve the community by decreasing disparities through navigation and education. Builds working relationships, solves problems, removes barriers to care and support patients while assisting them through the continuum of care. Educates people within the community on, disease, prevention, and necessary health screenings. Coordinates appointments and testing for early detection of medical issues. The Community Outreach Patient Navigator identifies and links eligible patients and their families to required screenings and services. Assists with removing barriers to care, such as insurance and financial needs, transportation, knowledge and other psychosocial concerns.

Responsibilities:

Coordinates and educates patients within the community on disease prevention and screeningIdentifies and addresses barriers to care, fear, trust, financial, insurance, transportation and psychosocial to ensure appropriate clinical care is coordinated and provided in a timely mannerHelp patients navigate our complex health care systemsCoordinates appointments related to health screenings i.e. mammogram results, colonoscopy results and pathology reportsNavigates patients to appropriate health care provider  within all Cleveland area  facilitiesIdentifies barriers to care  and coordinates  resolution to including but not limited to

                   Social work support

                   Medicaid and Medicare

                   Prescription Assistance Programs

                   Community resources (gathering place & 211)

                  Connecting patients to primary care and medical homes

Support coordination and execution of  community events 

Education:

High school diploma required.

Languages:

Spanish (orally and written) preferred

Certifications:

None Required

Competencies (Complexity of Work):

Knowledge of Electronic Medical Record (EMR) preferredAbility to deal effectively with a variety of people and work in a team environmentExcellent patient service orientation skillsMust possess strong oral, written and organizational skills

Work Experience:

Minimum three years related experience in customer relations with diverse patient/culture populations, exhibiting problem solving skills.Excellent interpersonal skills with special emphasis on oral and written communication, process improvement, and problem resolution.Requires strong communication, analytical, and critical thinking skills.

Physical Requirements:

May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating. May need to remain still for extended periods, including sitting and standing.May need to lift boxes or carry objectsWorking evenings and weekends is required
