Community Resource Representative

The Outreach Specialist is responsible for conducting community outreach at locations including, but not limited to health centers, provider offices, community sites, schools, faith-based organizations, and other social service sites aimed at promoting programs of the Community Health Department. The Outreach Specialist will also be responsible for developing relationships with key community contacts, maintaining outreach logs, and following up with potential clients.

Required Qualifications:

Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. 1 year experience working with community groups or employer groups in a collaborative setting. Experience working with communities from diverse age, cultural and ethnic backgrounds.

Preferred Qualifications:

Bachelor's Degree preferably in a healthcare/public health-related field.
