Job Title: Community Response Clinician (2nd Shift)
Job Description

We are seeking a dedicated Community Response Clinician to join our team. The ideal candidate should have experience in case management, particularly with substance use disorders, mental health issues, and homelessness. Familiarity with Hartford, CT resources is preferred. The role involves supporting law enforcement, managing crises and emergencies, and working in a fast-paced, high-stress environment. Candidates must be able to work holidays and during inclement weather, and collaborate effectively within a diverse team.

ResponsibilitiesManage and document participant caseloads, ensuring timely updates and adherence to confidentiality protocols.Participate in audits, maintain infection control practices, and ensure compliance with HIPAA and other mandatory training.Facilitate referrals to external programs and services, and maintain comprehensive records of participant progress and discharge summaries.Orient new participants to treatment programs and provide information about agency services.Maintain ongoing communication with community providers and disseminate information about local support meetings.Attend all required trainings and meetings, and conduct presentations as needed.Complete all necessary documentation, including progress reports and summaries.Essential SkillsBachelor’s Degree in social work or related field.Valid driver's license.Three years of experience working with marginalized populations and case management.Additional Skills & QualificationsProficient in databases, MS Office, written reports, and maintenance of case notes.Strong organizational, motivational, time management, and interpersonal skills.Specialized training in crisis intervention, substance use and/or mental illness, domestic violence, and other related fields is preferred.Experience working with participants from diverse ethnic and economic backgrounds, with the ability to interact and communicate effectively.Knowledge of Human Services, Criminal Justice, Court systems, substance use, and target populations is essential.Thorough understanding of co-occurring disorders and social services in Hartford, with the ability to work in collaboration with other service providers.Work Environment

This position requires availability for the 2nd shift, Tuesday to Saturday from 1pm to 10pm. The role involves working in high-stress environments, similar to emergency room settings, and requires flexibility to work during holidays and inclement weather.

Pay and Benefits

The pay range for this position is $30.00 - $30.00/hr.
