Community Response Clinician 2nd Shift
Job Title: Community Response Clinician 2nd ShiftJob Description We are seeking dedicated and experienced professionals to join our HEART team as Community Response Clinicians. These roles will provide critical support and intervention for individuals experiencing mental health crises and substance abuse issues. The positions require work during the 2nd shift, from Tuesday to Saturday, 1pm to 10pm. Responsibilities + Provide brief immediate crisis intervention. + Deliver acute counseling services until participants engage in a treatment program or no longer need services. + Assist and educate in activities of daily living, support for employment/training search, and job coaching. + Facilitate strong innovative, culturally diverse thinking and aptitudes to adjust to the population and interests of the group. + Educate participants on risk factors of behaviors to facilitate change in thinking. + Assess and provide diagnostic evaluations to patients and referral sources. + Monitor participants/clients in the facility. + Ensure adherence to infection control practices. + Participate in regulatory audits and/or funders. + Manage a fluctuating caseload of active/discharged participants on a daily and timely basis. + Ensure referrals and support services are made to outside programs for case management and medical intervention if applicable. + Orient new participants to treatment programs. + Refer participants to the agreed-upon treatment program and participate in the initial treatment planning with the identified treatment provider. + Provide participants with information about other agency programs. + Develop, implement, and evaluate a comprehensive crisis plan with each participant, maintaining regular contact until participants engage in treatment/community resources or decline need for further assistance. + Re-assess participants to formulate treatment/community resource needs. + Provide weekly office visits to program participants as applicable. + Maintain follow-up contact and documentation requirements for every program participant. + Maintain participant/client confidentiality. + Provide ongoing liaison, referral, and advocacy for participants. + Attend all trainings and meetings as assigned for professional development. + Participate with community providers as it relates to program content areas. + Complete referral progress reports and/or closing/discharge summaries. + Maintain and disseminate listings of local AA, NA, and CA meetings as needed. Essential Skills + Bachelor's Degree in social work or related field. + Driver's license. + Three years of experience working with marginalized populations and case management. + Proficient in databases, MS Office, written reports, and maintenance of case notes. + Strong organizational, motivational, time management, and interpersonal skills. + Experience working with participants from diverse ethnic and economic backgrounds with the ability to interact and communicate effectively. + Knowledge of human services, criminal justice, court systems, substance use, and target populations. + Thorough knowledge of co-occurring disorders and social services in Hartford with the demonstrated ability to work in collaboration with other service providers. Additional Skills & Qualifications + Experience in crisis intervention and motivational interviewing. + Attend a minimum number of credentialing hours of substance use related training per year. + Maintain compliance with yearly mandatory training including confidentiality and HIPAA compliance. + Conduct presentations regarding program resources at assigned locations. + Flexibility to work in various settings, including participants' places of residence. + Ability to work evenings, weekends, and other shifts as assigned. Work Environment This role operates in a fast-paced, high-stress environment similar to ER work. The clinician must be able to work holidays and in inclement weather. The position involves working within a diverse team and requires the ability to troubleshoot problems quickly. The work environment may include various settings such as participants' places of residency, and it demands a high level of flexibility and adaptability to meet the needs of the community. Pay and Benefits The pay range for this position is $30.00 - $30.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
