This Community Service Professional 3 position is for a vacancy in our Developmental Disabilities Services (DDS) section located in Hammond, Louisiana. The incumbent may be required to travel into personal homes, agencies, and facilities in the Florida Parishes Human Services Authority (FPHSA) five parish area for the purpose of assessment, implementation and monitoring of services and supports. The five parish area includes Tangipahoa, St. Tammany, Livingston, St.Helena, and Washington parishes.

This position may be filled as a Probational Appointment. Applicants may be asked to serve a probationary period that may be anywhere from 6 months to 24 months.
Salary may be negotiable depending on the chosen applicant's level of experience. The minimum and maximum pay for the position is listed below:
+ Community Service Professional 3: $22.52/hour - $40.53/hour maximum

Minimum Qualifications
MINIMUM QUALIFICATIONS:
Six years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
Six years of full-time work experience plus three years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
A bachelor's degree plus three years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
An advanced degree in a social sciences field, nursing, public health; public, hospital, or health administration; speech-language pathology or communication sciences and disorders; physical, occupational, recreational, or music therapy plus two years of experience in providing health or developmentally disabled services, counseling, health care management, or social services.
EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

FUNCTION OF WORK:
To provide advanced level implementation, coordination, monitoring and evaluation of programs within offices specializing in services for those with developmental disabilities.
LEVEL OF WORK:

SUPERVISION RECEIVED:
Broad from a Community Services Supervisor, a higher-level professional or an administrator.
SUPERVISION EXERCISED:
May provide functional supervision over lower level Community Services Professionals.
LOCATION OF WORK:
Department of Health and Hospitals (DHH) and Human Services Districts/Authorities statewide.
JOB DISTINCTIONS:
Differs from the Community Services Professional 2 by the broad supervision received and the presence of more independent and complex assignments.
Differs from the Community Services Supervisor by the absence of direct-line supervision of professional and support level staff.
Examples of Work
The incumbent will be responsible for planning, organizing, and coordinating five major programs in the five parish area that FPHSA serves: Tangipahoa, St. Tammany, Livingston, St. Helena, and Washington. These programs include court, custody, residential placement, Intermediate Care Facilities (ICF/DD - community homes) certification, and transition services for individuals transitioning out of a Supports and Services Center.
This position will serve as a resource center to the general public in providing information about FPHSA and community resources.
Duties may include, but are not limited to:
-Compiles and maintains a list of community homes in the FPHSA area.
-Receives referrals from individuals, community homes, and other state agencies requesting an out of home residential setting. Conducts interviews with the applicant and family to review the service request and to assess needs.
-Educates individual and family regarding available and appropriate residential facilities for placement.
-Manages the admission or transfer approval by review of documents of information on individuals requesting community home admission and determines approval for Medicaid certification.
-Reviews professional reports and determines if required information is present or if additional assessments are required.
-Receives the request for the Entry process and coordinates the appointment for the eligibility process to begin. Interviews the individual/family and conducts the required assessments.
-Represents FPHSA in court proceedings when an individual with developmental disabilities must have a developmental disability professional present. Manages caseload of individuals with developmental disabilities who have any court actions.
-May be assigned duties associated with disaster/emergency preparedness such as the Medical Special Needs Shelter (MSNS), Command Center, Continuity of Operations (COOP), etc., as deemed necessary.
