Community Support Provider - Oshkosh Job Details Level Undisclosed Job Location Oshkosh Office - Oshkosh, WI Remote Type N/A Position Type Full-Time/Part-Time Education Level High School Salary Range $15.50 - $16.50 Travel Percentage up to 80% Job Shift Day Job Category Health Care Description The CSP assists with the day-to-day needs of clients in their home, the community, and employment settings. They carry out responsibilities in the following functional areas: client rights, personal cares, daily living activities, job coaching and regulatory compliance.     ESSENTIAL FUNCTIONS AND DESIRED OUTCOMES: + Performs duties relating to the support, care, welfare, supervision and training of persons with mental illness, developmental disabilities, and/or acute, chronic or long-term conditions. + Serve as a role model and coach for clients regarding developing and using effective problem solving, decision-making, conflict resolutions, independent living skills and responsibilities. + Provide supportive home care, independent living skills and development needs of clients to assist clients in implementing goals and objectives for meal planning and preparation, housekeeping, laundry, proper use of household appliances and furniture, shopping, budgeting, etc. Provide training, demonstration, and observation both individually and in group sessions about identified areas. Make accommodations as needed to assure client and staff safety. Provide assistance if client is not able to complete a task. + Provides personal cares as directed by individual needs in a client specific manner to include, but not limited to: personal hygiene, toileting, and independent living skills. + Follows safety and regulatory procedures in all areas of food preparation, serving of meals, menu planning, individual dietary restriction and cleanup and needs and preferences to ensure client health. + Monitors and documents medications of clients to ensure the client is administering their medications as prescribed. + Follows all documentation and reporting policies and procedures for any medication and/or safety issues, and significant events. + Coaches, monitors, and reports client’s verbal and physical behaviors to ensure the safety and health of clients and others. + Assists, escort, and provide physical and other types of assistance needed by client during social and recreational activities. + Teaches and guides clients in achieving hopes, dreams, goals and objectives through self-direction, decision-making, and goal setting. + Advocates, enforces, and monitors the protection of client rights to ensure the proper treatment of clients.  + Ensures continuous information exchange between staff, case managers, guardians, family members, funding agencies, employers and service providers, this is done by phone, email and meetings as required. + Provide individual services to the clients that are outlined in their care plan, following the authorized hours. + Provide support and supervision to persons with disabilities in their community work environment. + Provide on-site support and maintain a close working relationship with clients and employers.  Requirements + Must possess a high school diploma/equivalent and/or related work/life experience. + Must be at least 18 years of age. + Must meet agency driving standards. + Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.
