The Community Support Specialist contributes to the development, implementation, monitoring, and updating of Consumers’ individualized habilitation plans; therapeutic rehabilitative skill development; implementation of Illness Management and Recovery (IMR) principles and interventions; coordinating and managing services, both mental health and physical health; crisis intervention, as needed. Provision/coordination of services aimed at active rehabilitation and support services to achieve restoration of functioning and the achievement of community integration; maintaining own appointment schedule and meeting work productivity requirements. Other duties as assigned by management staff. Productivity requirement of 60% face to face contact weekly. Must be able to operate an Agency vehicle and maintain a driving record acceptable to Agency’s insurance provider.
