OverviewThe CSS is an essential part of the Independence Program (IP) team reporting directly to ResCare Home Care, as well as the Independence Program Case Manager. This is a community-based position working with Veterans who have brain injuries, PTSD, neurological disabilities and other cognitive or physical challenges. While providing functional support to a Veteran Warrior, the CSS is responsible for providing opportunities for increased participation in community activities, increased social capital, improved quality of life and development of meaningful and purposeful activities.
This is a PRN opportunity. Each Veteran Warrior is typically authorized for 10 to 15 hours per week with their Community Support Specialist. More than one Veteran Warrior may be assigned to a Community Support Specialist if availability allows for adequate scheduling for the Veterans and their CSS.
External Job Description Provides positive support and be an advocate for the Veteran WarriorAids with identifying choices and making informed decisions that involve their activities and lifeProvides structure and support in household management such as: maintaining cleanliness of home environment and establishing a safe, healthy, and efficient routineAssists with participation in recreation and community activities promoting socialization and establishing relationships with peersAssists Veteran Warrior with improving community living and integration skills in a variety of settings such as shopping, church attendance, sporting events, recreation, club participation, classroom support, volunteer/employment support, etcProvide training or assistance with mobility in home environment, using adaptive aids, and accessing public transportationHelps with developing effective and appropriate communication and social skillsBased on the established goals of an individualized Care Plan provides Veteran Warriors functional supports to acquire, retain and improve independenceIdentifies barriers, triggers, or changes in functional status to develop proactive interventions that promote continued independenceEducates Veteran Warriors on how to develop necessary problem-solving skills and strategiesServes as a role model/mentor and maintains good communication/relationships with the Veteran Warrior, their families and referral sourcesCompletes visit progress notes and timecards in compliance with organization policies and submits these documents as instructed and in a timely mannerResponsible for monitoring and appropriately utilizing billable hoursExhibits professional behavior at all timesParticipates in required training through ResCare and respective referring partners Communicates regularly with organization personnel regarding issues/concernsParticipates in team meetings as requestedPerforms other duties as assigned to meet the program goals or as requested by ResCare and respective referring partnersMaintains organization confidentiality of information relating to individuals and their families in accordance with HIPPA and PHI/company policiesOther duties as assigned Qualifications Bachelor's Degree in related field and experience listed below OR Associates Degree with one year experience in related field and experience listed below OR High School Degree/Equivalency with three years’ experience in related field and experience listed belowDemonstrated knowledge of brain injury, accessibility to community resources and experience with appropriate training skills, assessment and evaluation skills preferred Certified Brain Injury Specialist (CBIS) preferredVeterans are welcome and encouraged to apply About our Line of BusinessAll Ways Caring HomeCare delivers quality, compassionate and highly individualized care and support that helps people in need of assistance stay at home – all while maximizing their dignity, privacy and independence.
