ResponsibilitiesResponsible for the overall physical, psychological and emotional health and safety of the participant.
Models healthy behavior and positive social skills.
Provides supports as written in the individual’s support plan that may include personal care, physical lifting and therapeutic holds.
Provides feedback to team members and/or supervisors as to effectiveness of individual plans.
Assists with an individual’s environment such as cleaning, laundry, shopping, cooking, errands and budgeting.
Facilitates and maintains family contacts and community involvement.
Encourages the development of new relationships and contacts by assisting people to be active in their communities and workplace.
Communicates effectively in verbal and written format. Communication is to be given and received and is every employee’s responsibility.
Actively participates in improving communication with participants by learning, encouraging and actively utilizing an individual’s modes of communication.
When applicable, follows doctors’ orders and ensures that medications and treatments are administered following proper medical procedures. Monitors the dietary needs of the program participant. Recognizes reports and assists with medical crises as they occur.
May assist in the vocational evaluation process.
May support vocational programming including payroll for hours worked, merit ratings, referrals and job coaching.
Performs work in compliance with Human Rights regulations.
May work independently of direct supervision or, at times, alone.
Maintains confidentiality and HIPAA compliance.
Maintains and takes responsibility for certifications and all trainings as required, respectful of management leadership at all times.
Contributes to the positive morale by representing the agency through professional communication and interactions.
Implements and fosters the principles of self advocacy which promote an independent lifestyle and respect for program participants.
Assists people with learning about and advocating for their personal rights and understanding the responsibilities that accompany those rights.
Follows all established policies and procedures and documents accordingly.
High school diploma or equivalency. Valid driver’s license with acceptable driving history. Able to communicate verbally, through touch, visually and in writing. Basic budgeting and mathematical skills. Proven ability to demonstrate tact, good judgment and initiative in interactions with participants, peers, family members and the general public.
