Description
Summary:
The COMPASS Community Wellness Advocate will work closely with HIV-positive patients, as part of an interdisciplinary team to help HIV positive patients address barriers to treatment adherence, viral load suppression and retention in care through home, field and clinic based visits.
Requirements:
+ Engage patients who are newly diagnosed and/or facing barriers to treatment adherence, viral load suppression and/or retention in care
+ Assists other program staff in screening all patients receiving HIV primary care at Institute sites for COMPASS program eligibility
+ Meets with enrolled and not-yet enrolled patients when they present for medical care, to engage in services and screen for needs
+ Carry out field and clinic based outreach, to re-engage patients to care
+ Follows program policies and procedures for assigned patients to ensure services are delivered according to program track
+ Assists in completing program intakes, reassessments, care plans, case conferences, adherence assessments and other program services/forms within specified time frames
+ Provides individual and group based health promotion, treatment adherence counseling, risk reduction counseling and case management to assist patients in decreasing barriers to viral load suppression and retention in care, through home, field and clinic based visits
+ Provides accompaniments to patients to healthcare and social service appointments
+ Screens patients for mental health and substance use treatment needs, refers for needed services
Qualifications
+ High School Diploma required
+ Person with lived experience with HIV preferred
+ Comfortable disclosing experience and history
+ Computer literacy with proficiency in MS Word, Excel, and PowerPoint.
+ Demonstrated organizational, interpersonal, oral and written communication skills and the ability to handle multiple assignments at any time
+ Ability to generate, interpret and analyze data from multiple sources
+ Knowledge of social services, particular for individuals with HIV, in the NYC area
+ Bilingual English/Spanish preferred.
