GENERAL SUMMARY

Provide transportation services for residents traveling to resident activities, doctor’s appointments, physical therapy, or other medical treatments.

Must be 21 years of age or older, have a valid Ohio driver’s license, must have maintained a driver's license for at least the previous two (2) years and must keep a clean driving record. Any points against license must be reported and could be cause for dismissal.

Must have a current card issued as proof of successful completion of the “American Red Cross” (or equivalent certifying organization) basic course in first aid, as well as a CPR certificate or EMT certification.

ESSENTIAL FUNCTIONS/POSITION SPECIFIC DUTIES

Presents identification card to the patient identifying his/her complete name and company affiliation.

Perform other duties as assigned or required to allow for the smooth and efficient operation of Senior Services resident transport.

Provides care that is observant and responsive to resident needs as they arise.

Responsible for completing required paperwork and documentation.

Knowledge of streets in the Norwalk area and ability to choose the best route to the destination and return to NMH/TCU/CH, considering road conditions, and resident comfort.

Abides by all traffic regulations and speed limits.
