This is an Hourly position. Overview POSITION SUMMARY: Our HDM Coordinator performs outreach services, makes home delivered meal assessments, provides nutrition education to home bound clients, and provide support services as needed in assigned program area. Qualifications REQUIRED QUALIFICATIONS: Education: Associates Other Licenses/Certifications: Drivers Lisence Other skills/knowledge: Excellent interpersonal and communication skills.
