**Job Summary and Qualifications**
Responsible for assisting in the supervision and delivery of consistent services and care of the patient who presents with or develops sepsis during their hospitalization at Wesley Medical Center or Wesley Woodlawn Hospital & ER. Assists in the organization of services and systems necessary, allowing and structuring a multidisciplinary approach in accordance with evidence-based care practices supported by literature. Serves as a resource in the clinical care of the patient with sepsis in collaboration with intensivists, hospitalists, and other care providers. Reviews, aggregates patient specific data, and care management for all sepsis patients, documenting care and responses, using data analysis tools to improve care. Provides a mechanism for the ongoing coordination of care among disciplines. Supports the organization's vision and mission statements. Utilizes knowledge of patient's age, cultural background, and social mores in the provision and planning of patient care. Contributes to the provision of quality nursing care through performance improvement techniques that demonstrate positive outcomes in patient care. This position reports directly to the Sepsis Program Manager.
+ Demonstrates the ability to adapt technique/treatment to age of population served, including newborn, pediatric, adolescent, adult and geriatric patients, while providing care/service with respect to cultural differences and spiritual needs
+ Plans the delivery of patient-centered care in accordance with standards of care defined by national consensus documents and Wesley Medical Center
+ Systematically evaluates the quality and effectiveness of the clinical care of the sepsis patient by constructing, managing, and driving quality of care activities directed at initiating, implementing and sustaining process improvements
+ Acquires and maintains current knowledge of sepsis disease management and treatment strategies
+ In partnership with the physician and nursing leaders, acts as an expert resource in the management and treatment of the sepsis patient
+ Participates and assists in leadership of an interdisciplinary quality collaborative directed toward improvement in outcomes in the patient experiencing sepsis
+ Monitors the sepsis patient population's progress toward the attainment of positive outcomes, individually and as a group
+ Applies quality-improvement data analysis tools to the presentation of outcome and process information to internal and external stakeholders
+ Collaborates with the patient, family, and significant others in providing and advocating for patient care improvements
+ Participates in the improvement of operational functions within the departments where sepsis patients are provided care
+ Establishes and maintains effective and respectful communication in development of all collaborative relationships
+ Evaluates and participates in opportunities for outstanding service to all customers and accepts responsibility in partnership that is equally respectful to everyone
+ Coordinates a team process and response to the emergent presentation of patients with sepsis, including early identification, the implementation of early goal directed therapy, and the integrated follow-up and communication of care throughout the patient's hospital stay
+ In coordination with the physician leadership and PRO team, conducts outreach education to upstream providers who send septic patients to Wesley Medical Center
+ Demonstrates the ability to utilize critical thinking and problem solving skills to identify, interpret, and access appropriate resources in patient specific scenarios
+ Demonstrates the ability to apply knowledge of age-appropriate competencies to specific patient populations
+ Participates in state and national level groups directed toward the improvement of care of the sepsis patient
+ Assist with the surveillance activities of the infection prevention program of the hospital directed toward the prevention of sepsis development within the patient population
+ Performs all other duties as assigned. What qualifications you will need:

Required Registration, Licensure and/or Certification: Current Basic Life Support (BLS) verification course through American Heart Assoc. or American Red Cross - Healthcare Provider CPR
Preferred Registration, Licensure and/or Certification"
+ Current Licensure in the State of Kansas as a Registered Nurse (RN) or Current Multi-State Licensure as a Registered Nurse with the ability to obtain a Kansas Registered Nurse license within 90 days of start date
+ Advanced Cardiac Life Support (ACLS) verification
Required Education or Experience:
+ Associate’s degree in Nursing or related discipline
+ Enrollment in a Bachelor’s degree program within 6 months of acceptance of position
Preferred Education or Experience:
+ Bachelor’s degree in Nursing or related discipline
+ Experience running project or programs, managing multidisciplinary teams, or working on process improvement committees or councils
+ Experience caring for sepsis patients
