Coordinator Oncology Care
Under the guidance of the Director, Cancer Center, the Coordinator, Oncology Care is responsible for coordination of multidisciplinary clinic programs and care continuum for patients diagnosed and/or to receive therapy at the Medical Center. + Coordinates multidisciplinary clinic programs including program development and services. + Navigates the care continuum for patients by collaborating with the Medical Director, referring physicians and other professionals. Acts as a ""compass"" for patients diagnosed and/or receiving therapy at the Medical Center. + Ensures timely scheduling of diagnostic work-up, results reporting and communications between patients and the multidisciplinary team. + Promotes the multidisciplinary team process and ensures patient receives necessary referrals and follow-up. + Provides efficient and effective care for patients and families utilizing resources efficiently and effectively. Keeps current with emerging technologies and treatment trends. + Coordinates cancer related patient/staff education programs, projects, services and groups. + Assists with community events such as health screenings and detection support. Evaluates the effectiveness and results of such programs and services and is involved with outcomes research. + Compiles and analyzes Quality Improvement data and benchmarking. + Serves as a patient advocate by promoting timely, competent and compassionate care. + Completes mandatory education and training in order to maintain organization and department specific competencies and requirements. Maintains applicable certification/licensure. Maintains required CPR (BLS). + Other duties as assigned **Education:** + BSN preferred + Program completion in professional nursing **Certifications:** + current RN license in state in which employed. + Oncology Certified Nurse (OCN) strongly preferred. + Maintains current Basic Life Support (BLS). + Any registered nurse or advanced practice nurse must obtain a cancer specific certification or demonstrate ongoing qualifying education within the timeframe of the facilities accreditation cycle, if they work in medical oncology, radiation oncology, cancer center or cancer clinic and/or administer chemotherapy within an accredited Cleveland Clinic facility. **Complexity of Work:** + Knowledge of oncology prevention diagnosis disease process and symptom management. + Ability to build and maintain effective relationships with patients and multidisciplinary teams. + Ability to effectively interpret analyze and respond to difficult patient care situations. + Understanding of adult and geriatric patient needs. + Effective written and verbal communication skills. + Proficient personal computer skills. **Work Experience:** + Five years nursing experience including two years in oncology required. + Experience in case management mental health program development and community health/home care preferred. **Physical Requirements:** **Personal Protective Equipment:** + Follows standard precautions using personal protective equipment as required Salaries [which may be] shown on independent job search websites reflect various market averages and do not represent information obtained directly from The Cleveland Clinic.
