Job Summary: The Coordinator-Palliative Care (Social Work) identifies patient and family psychosocial and environmental needs related to home care environment within the Transitional Care Management framework. Works collaboratively with other members of the interdisciplinary team in developing and implementing a comprehensive, integrated discharge plan. Makes appropriate referrals for community services; may provide emotional support and counseling services, and crisis intervention. Assesses and intervenes in situations involving child or elder abuse/neglect, domestic violence. Provides care and services to adult and elderly patients. Performs other duties as needed.
Education and Experience: Master's Degree in Social Work required. Minimum one year of experience in clinical setting required. Previous home care or case management experience preferred.
Knowledge and Skills: In-depth knowledge of case management practice in relation to transitions of care. Able to read; write legibly; speak in English with professional quality; use computer, printer, and software programs necessary to the position; operate/troubleshoot basic office equipment; type 30 words per minute without errors. Able to relate and communicate positively, effectively, and professionally with others; provide leadership and supervision; follow policies consistently; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to positively and effectively manage complex interpersonal relations with patients and staff. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; work independently and with minimal supervision; perform basic math functions; manage multiple assignments effectively; compose written material; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Works independently without direct supervision. Able to distinguish colors as necessary; write, hear, read and speak sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with workplace; see adequately to read computer screens and written documents necessary to the position.
Licensures and Certifications: Licensed Clinical Social Worker (LCSW) required.
