CPST Substance Use Specialist

Summary:

The CPST (Community Psychiatric Support Team) Substance Use Specialist provides support to adults with addiction and mental health concerns to help them improve their functioning within their communities. The CPST Specialist is responsible for 1) coaching clients in coping strategies; 2) improving relationships with family and friends; 3) learning skills to manage symptoms and improve daily living; and 4) coordinating care with other providers including primary care and 5) connecting clients with community resources for support with social drivers and for successful discharge planning. 

 

Responsibilities:

Assists SUD counselors in conducting screenings and assessments of clients through the person-centered treatment planning process.

Assists clients and family members or other collaterals with identifying strengths and needs resources natural supports and developing strategies to use them effectively.

Assists clients individually and in IOP groups to identify behaviors associated with their substance disorder which interfere with their daily living and community integration.

Provides supportive intervention to help the clients identify and apply coping strategies.

Participates in the Center-based team observation support and supervision of clients experiencing withdrawal symptoms

Provides individual and group interventions as requested by SUD Manager

Coordinates community resources to address financial housing food benefits employment and transportation needs.

Communicates with internal and external providers to foster coordination of care.

Assists nurse in supporting clients manage chronic medical conditions establishing good health routines and practices personal care and accessing medical care.

Maintains knowledge of available providers and resources in the community to address specialized needs and for successful discharge planning.

Participates in supervision team conferencing department and agency meetings.

Maintains detailed accurate and timely documentation in health record.

Enhances and maintains professional expertise through educational activities and trainings.

Meets requirements for client contact time according to treatment plan and for agency productivity targets.

Follows agency policies and standards for client rights and confidentiality.

Perform other duties as assigned.

 

Other information:

QUALIFICATION REQUIREMENTS:

Associate�s degree in a human service field such as psychology counseling education rehabilitation.

At least 2 years experience working with in a behavioral health or human service setting

NECESSARY SPECIAL REQUIREMENTS:

Knowledge of human development and family systems issues.

Understanding of mental health and substance use diagnoses

Knowledge of community resources for clients and families with behavioral health disorders.

Advocacy and coaching skills.

Valid driver's license and use of in order to perform the major functions of the job.

Automobile liability insurance coverage for vehicle being used in accordance with Center requirements.

Ability to work collaboratively as a team to coordinate care for clients with intensive behavioral health needs.
