Credentialing Coordinator

Job Description:

Credentialing Duties and Paperwork:
• Coordinates and facilitates credentialing and re-credentialing process for all Physicians and Allied Health. Processes additional privilege and status requests for MD’s and AHP’s as necessary to include out of cycle, reinstatements, and temporary privileges.
• Communicates with Baptist Health CVO on applications for pre-approval and processing of all initial physician and allied health professional applications.
• Communicates with applicant for completion of application process or additional documentation of privilege clarification.
• Process Affiliate applications.
• Facilitates process for submitting documentation to IT to create Epic ID user accounts for Initial Applicants.
• Process resignations as well as reinstatements for Physicians and AHPs
• Process AHP change of employment or supervising physician
• Gather in-house documentation for review at reappointment of physician’s clinical activity, including but not limited to data for peer review activity.
• Facilitates the FPPE provisional review process for all physicians at their twelve-month anniversary and places into alpha cycle.
• Generates all Administrative Board approval letters as they relate to Physician and AHP credentialing upon appointment and reappointment to include updating delineations and rosters. Maintains communication of initial appointments and reappointments with in-house directors and staff.
• Facilitates updating and maintenance of all delineations in accordance with medical staff bylaws and policies to include researching privileges for changes.
• Communicates with CVO to facilitate changes and updates to credentialing forms ensuring consistency with Baptist Health Lexington requirements and ACHC standards.

Medical Staff Functions:
• Assures compliance of Medical Staff and Bylaws, Policies, and Credentialing Manual to meet hospital and national credentialing agencies requirements.
• Facilitates updating/maintaining the Baptist Health Lexington Credentialing Procedures Manual.
• Assists the department director to update medical staff policies relating to credentialing.
• Responsible for conducting new AHP orientation and assist with new Physician orientation when needed.
• Assist medical staff members and allied health professionals by providing direction for questions, concerns, or complaints.

Facilitates Credentials Committee meetings including proper documentation and follow-up:
• Prepares finalized Agenda, attends and records actions of the Credentials Committee. Responsible for all follow-up at conclusion of meeting.
• Prepares credentials report for MEC and Board meeting.
• Upload’s agenda packet and supporting documents.

Carries out responsibilities of various committee or departmental meetings, which include minutes, agenda, attendance, and follow-up:
• In addition to Credentials Committee, back-up for other meetings as necessary.
• Provides support for annual General Staff meetings.
• Responsible for taking Credentials Committee minutes and prepares in hospital format and finalizes information as requested by committee.
• Responds to hospital follow-up as requested by Committee.

Demonstrates accountability for accurate data input on all physician and Allied Health information disseminated to the various software programs and hospital departments:
• Ability to run established reports in CredentialStream as needed for credentialing or other MSO functions.
• Builds reports, filters, privileges, and workflows in CredentialStream.

Ensures positive customer relations interactions:
• Answer’s telephone and follow problems to solution by providing good customer service to all callers.
• Provides assistance as needed to walk-in visitors or customers.
• Directs individuals with physician, AHP or hospital complaints to the appropriate individuals for follow-up.
• Completes post MEC data entry.
• Completes and distributes New Staff Memo to appropriate hospital staff.

Support for Documentation Requests and Other Duties:
• Provide support for CMO and Department Director as directed.
• Provides coordination for requests to gather information from Decision Support and disseminate based on policy.
• Provides updates to KBML for PA and Allied Health Supervising Physician listing (monthly updated) and reported Quarterly to KBML.
