Credentialing Coordinator - Hospital Based Programs
Job Description

CHS operates 71 acute-care hospitals and more than 1,000 other sites of care, including physician practices, urgent care centers, freestanding emergency departments, occupational medicine clinics, imaging centers, cancer centers and ambulatory surgery centers.

Essential Duties and Responsibilities:

Enter provider information into the credentialing database, confirm CV is in the correct date/year format, and initiate onboarding.

Review information and documents submitted by the providers to ensure documents are not expired, are clear and saved in the appropriate folders.Complete primary source verifications to ensure the provider meets APP standards and escalates any red flags as per the standard operating procedures.Entry of collected data during the onboarding process into the credentialing database.Gather providers CAQH and NPI Taxonomy code information and enter into the credentialing database.Escalate concerns regarding the processing of providers and non-response to management.Assist credentialing team with start-up and acquisition data entry and document collection.Follow up with providers on 180-day, 90-day, and 60-day expirables to ensure timely renewal.All other duties as assigned.

Minimum Qualifications:

High School diploma or equivalent required.Associates degree preferred.Customer service and/or administrative experience preferred.Basic computer and keyboarding skills required.
